DESSA PDF Forms for Sharing/Review
Overview
This article lists the watermarked PDF forms for each of the DESSA assessments to provide to parents/guardians, community members, or other key stakeholders for informational and review purposes as needed. Each form is watermarked confidential and may be used to do things such as post the forms on the district website to make them available to parents or attach them to a parent letter.
📌 Note: The assessment forms (which are no longer available on our support portal site) are meant to be used for completing ratings and should not be used for informational purposes such as posting them on district websites or sharing them in parent letters. If you need to share out the DESSA assessment forms for informational purposes, please contact support@apertureed.com for the watermarked versions of the rating forms listed below.
