Overview

This article provides downloadable PDF forms for each of the DESSA Second Step Assessments. The forms are paper versions of the online rating forms that are typically completed by educators in the DESSA System.

Appropriate raters for each DESSA Second Step Assessment include:

  • DESSA-Second Step Edition (DESSA-SSE): Educators and Parents/Guardians
  • DESSA-Second Step Elementary School Edition (DESSA-SSESE): Educators
  • DESSA-Second Step Middle School Edition (DESSA-SSMSE): Educators
  • DESSA-Second Step Middle School Edition Student Self-Report (DESSA-SSMSE SSR): Students in Grades 6-8

Note: These forms are meant to be used for completing ratings and should not be used for information purposes such as posting them on district websites or sharing them in parent letters.
